HGV/LGV Medical
For the medical please bring along with you your D4 medical form which can be found on :

We would be grateful if you could complete the following sections;
‘Your Details’ (Applicant)
‘Your Doctors Details’ on page 1
Name and date of birth on the bottom of every page and bring the forms along to your appointment with you.
When attending the appointment we will require:
- Your driving license.
- If you take any medication or are under hospital treatment please bring this with you. If you have diabetes or heart problems we will need a print out of this specific health information from your GP.
- If your wear contact lenses please remove these 24 hours before your appointment and bring along your glasses.
